Overview:
The main focus of this position is to coordinate client care with referral source, client and employee, ensuring the quality services delivery.
This coordination occurs through the direct supervision of employees, assessments with clients, and ongoing communication with referral sources.
All coordination must be conducted within contract, state and federal regulations, as well as Company Policies and Procedures.
This position reports directly to the Branch Manager.

Responsibilities:
· Perform On-the-Job Training (OJT) for PCA field staff;
· Perform on-site evaluations for all field staff;
· Complete required documentation;
· Complete all annual evaluations and submit to Branch Manager;
· Work open shifts, as directed by Branch Manager, as well as work shifts regularly assigned, not to exceed forty (40) hours per week unless approved by the area manager;
· Assist Branch Manager in teaching the training classes, as requested;
· Assist Branch Manager with on-site investigations, as requested, for worker injury and client complaints;
· Assist Branch Manager with filing and file compliance, as directed and approved;
· Assist with reference checks, EDL verifications, and interviews, as directed by Branch Manager;
· Set up new employee files as directed by Branch Manager;
· Assist staffing coordinator as directed by Branch Manager;
· Participate in on-call rotation, as assigned;
· Maintain and promote professional manner for self, field and office staff, and during telephone communications;
· Monitor phone calls and perform traffic control in the office;
· Communicate with Staffing Coordinator, Branch Manager, and Case Worker, as needed and/or directed by Branch Manager;
· Document on employment calendars when needed;
· Complete communication form as applicable; AND
· Assist Branch Manager with other duties, as assigned and with approval.
Minimum Qualifications:
· At least two (2) years supervisory experience in programs dealing with elderly and disabled individuals and meet one (1) of the following requirements:
a.
A Bachelor’s Degree in Social Work, or a related profession, with one year of direct experience working with aged and disabled participants,
b.
A Licensed Registered Nurse (R.
N.
) or Licensed Practical Nurse (L.
P.
N.
), with one year of direct experience working with aged and disabled participants, and two years of supervisory experience, or
c.
A high school diploma with four years of direct experience working with aged and disabled participants, and two years of supervisory experience
